- Medical Evaluation Questionnaire (MEQ) is approved for N95 and/or Respirator appts.
- Will report clean shaven for ALL appts.

Please read the following prior to scheduling:
To request a copy of your Gas Mask or current Respirator fit test please call 707-423-5490.
Respirator Fit Tests:
A one time Medical Evaluation Questionnaire (MEQ) is required to be completed with Travis AFB Public Health Flight prior to Respirator Fit Testing. If there is no verified signed MEQ on file, you will be turned away until an MEQ is completed.
NOTE: This is NOT required for Gas Mask fit testing.
Gas Masks:
Gas Mask fit tests are once per career unless any of the following has occurred since your last fit test:
- Major facial scarring/surgeries
- Weight loss/gain of at least 10%
- Changes in make/model/mask sizing (This applies currently to members last tested in 2012 and prior)
